Luminum Vitality

Aging well today often means juggling multiple specialists, a growing list of medications, and long stretches between checkups with no one connecting the dots in between. A single annual wellness visit and a string of brief specialist appointments leave many older adults, and the families supporting them, without a consistent point of contact for day-to-day questions, medication concerns, or simply someone keeping watch.

Vitality provides ongoing clinical guidance, structured support, and personalized care planning to help members manage chronic conditions, stay independent, and navigate the healthcare system with one responsive, dedicated care team behind them.

Older adults managing chronic conditions often see a primary care provider a few times a year and separate specialists on separate schedules — with no one connecting the dots in between. Questions go unanswered until the next appointment, medication lists grow without review, and small changes can go unnoticed until they become a hospital visit.

Vitality provides one coordinated care team focused on keeping members healthy, independent, and supported between traditional healthcare visits — and keeps families informed along the way

Disclaimer
This program does not replace primary care, specialist, or emergency care services. Luminum Health provides non-emergency chronic care management, healthy-aging support, and care coordination. Prescriptions and laboratory testing are provided only when clinically appropriate and within applicable state regulations.

Questions About Luminum? Start Here

What is Luminum Health?

Luminum Health is an online, clinician-led health membership designed to provide personalized support for postpartum recovery, PCOS, sleep, anxiety, depression, skin health, and overall wellness. Members can choose their program and receive guidance from a dedicated care team, including a Registered Nurse, Family Nurse Practitioner, and Care Coordinator.

How does the program work?

Start by choosing the program that fits your health goals. You’ll complete a secure online health assessment, and your care team will review your information to create a personalized care plan. From there, you’ll receive ongoing support through video visits, secure messaging, and care guidance tailored to your needs.

What’s included in the $199/month membership?

Your membership includes ongoing clinician-led support, secure messaging, care coordination, personalized guidance, and scheduled video visits with your care team. Depending on your program and clinical needs, your Family Nurse Practitioner may also support labs or prescriptions when appropriate.

Who reviews my health information?

Your health information is reviewed by licensed clinical team members, which may include a Registered Nurse and Family Nurse Practitioner. Your Care Coordinator may also help with scheduling, follow-ups, and care navigation.

Can Luminum prescribe medication or order labs?

Yes, when clinically appropriate. A licensed Family Nurse Practitioner may order labs or prescribe medication based on your health history, symptoms, program goals, and state-specific clinical requirements. Prescriptions are not guaranteed and depend on medical review.

Can I message my care team between visits?

Yes. Members can use secure messaging to ask questions, share updates, and receive guidance between scheduled visits. This helps your care team stay connected to your progress and adjust support as needed.

Can I cancel my membership?

Yes. Luminum is a monthly membership, and you can cancel before your next billing cycle. Your access and care support continue through the end of your active membership period.
