Welcome to Luminum Health. These Terms & Conditions explain the rules for using our website, online services, membership programs, and related care support. By accessing our website, creating an account, submitting information, or using our services, you agree to these Terms.
If you do not agree with these Terms, please do not use Luminum Health services.
1. About Luminum Health
Luminum Health provides online, clinician-led care programs for eligible members in supported states. Our programs may include health assessments, care coordination, secure messaging, video visits, wellness guidance, lab support, and prescription support when clinically appropriate.
Luminum Health is not an emergency medical service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
2. Eligibility
You may use Luminum Health only if:
- You are at least 18 years old;
- You live in a state where Luminum Health is available;
- You provide accurate and complete health and contact information;
- You agree to use our services only for yourself unless otherwise authorized; and
- You understand that care is subject to clinical review and may not be appropriate for everyone.
We may decline, pause, or discontinue services if we determine that Luminum Health is not clinically appropriate for your needs.
3. Not a Substitute for Emergency or In-Person Care
Luminum Health provides online support and care guidance. Our services are not a substitute for emergency care or urgent in-person medical evaluation.
Do not use Luminum Health for emergencies, including but not limited to chest pain, trouble breathing, severe bleeding, thoughts of self-harm, sudden weakness, severe allergic reaction, loss of consciousness, or any condition that may require immediate medical attention.
If you are in crisis or may harm yourself or others, call 911, call or text 988, or seek immediate emergency help.
4. Clinical Services and Care Team
Depending on your program and state availability, your care team may include a Registered Nurse, Family Nurse Practitioner, Care Coordinator, or other licensed or administrative support team members.
Clinical services are provided only by appropriately licensed professionals. Care Coordinators and other non-clinical team members may help with scheduling, reminders, forms, communication, and care navigation, but they do not diagnose, treat, or prescribe.
5. No Guarantee of Diagnosis, Treatment, Prescriptions, Labs, or Results
Submitting information or becoming a member does not guarantee that you will receive a diagnosis, prescription, lab order, medication, referral, or specific treatment plan.
Prescriptions, labs, and clinical recommendations are provided only when a licensed clinician determines they are medically appropriate. Individual results vary, and Luminum Health does not guarantee any specific health outcome.
6. Membership and Payment
Luminum Health may offer monthly membership programs. Pricing, program features, and availability may vary by program and state.
Unless otherwise stated, membership fees are billed monthly and automatically renew until canceled. By providing a payment method, you authorize Luminum Health or its payment partners to charge applicable fees.
You are responsible for keeping your payment information current.
7. Cancellation and Refunds
You may cancel your membership before your next billing cycle. Cancellation stops future billing but does not automatically refund prior charges.
Refunds, if any, are handled according to Luminum Health’s refund policy and applicable law. We may offer a trial, promotional period, or satisfaction guarantee only when clearly stated on the website or in writing.
Your access to services may continue through the end of your paid membership period unless otherwise stated.
8. Insurance
Luminum Health is currently designed as a cash-pay membership service unless otherwise stated. We may not bill insurance, Medicare, Medicaid, or other third-party payers.
You are responsible for all membership fees and any third-party costs, including labs, medications, pharmacy charges, or outside provider services, unless specifically included in your program.
9. Third-Party Platforms and Partners
Luminum Health may use third-party platforms and service providers to deliver services, including electronic health record systems, telehealth tools, secure messaging, scheduling, payment processing, laboratories, pharmacies, and technology vendors.
For example, Luminum Health may use Healthie or similar healthcare technology platforms to support scheduling, forms, messaging, telehealth visits, and health records.
Your use of third-party platforms may also be subject to their own terms, privacy policies, and security practices. Luminum Health is not responsible for third-party service interruptions, technical issues, or policies, but we aim to use vendors that support secure and compliant healthcare operations.
10. Privacy, HIPAA, and Protected Health Information
Luminum Health takes privacy seriously. Health information you provide may be protected under the Health Insurance Portability and Accountability Act of 1996 and related regulations, commonly known as HIPAA.
Our use and disclosure of protected health information is described in our Notice of Privacy Practices. Our website privacy practices are described in our Privacy Policy.
By using Luminum Health, you agree that we may collect, use, store, and share information as needed to provide services, operate our business, coordinate care, process payments, comply with law, and support healthcare operations.
Where required, Luminum Health uses written agreements with vendors that handle protected health information on our behalf.
11. Secure Messaging and Communication
Luminum Health may offer secure messaging through its care platform. Secure messaging is intended for non-emergency questions, updates, and care coordination.
Response times may vary. Secure messages should not be used for urgent or emergency medical needs.
You agree that Luminum Health may contact you by email, phone, SMS/text message, patient portal message, or other communication methods for scheduling, account, billing, program, and care-related purposes. Standard messaging or data rates may apply.
12. Telehealth Consent
By using Luminum Health services, you consent to receive care through telehealth, which may include video visits, audio communication, secure messaging, remote review of forms, and electronic health information.
Telehealth has benefits, including convenience and access to care, but also has limitations. A clinician may determine that telehealth is not appropriate for your condition and may recommend in-person evaluation or emergency care.
13. Your Responsibilities
You agree to:
- Provide accurate, complete, and current information;
- Update Luminum Health if your symptoms, medications, allergies, pregnancy status, health conditions, or contact information change;
- Use medications, labs, and care guidance only as directed;
- Ask questions if you do not understand your care plan;
- Seek emergency care when needed;
- Keep your account login information confidential; and
- Use Luminum Health services lawfully and respectfully.
You are responsible for decisions you make based on your care plan and for following up with other providers when recommended.
14. Account Security
You are responsible for maintaining the confidentiality of your username, password, and account access. Please notify us immediately if you believe your account has been accessed without authorization.
Luminum Health is not responsible for harm caused by unauthorized access resulting from your failure to protect your login information.
15. Website Content
Content on the Luminum Health website is for general informational and educational purposes only. It is not medical advice and does not create a provider-patient relationship unless and until you complete required onboarding steps and are accepted for clinical services.
Website content may be updated or changed at any time without notice.
16. Program Availability
Luminum Health services are available only in supported states and may vary based on clinician licensure, state laws, clinical appropriateness, and operational availability.
We may modify, suspend, or discontinue programs, features, pricing, or availability at any time.
17. Acceptable Use
You agree not to misuse the website, platform, or services. You may not:
- Provide false or misleading information;
- Use another person’s account without authorization;
- Harass, threaten, or abuse staff or clinicians;
- Attempt to access systems or data without permission;
- Interfere with platform security or operations;
- Use Luminum Health for unlawful purposes; or
- Copy, reproduce, or misuse Luminum Health content, branding, or materials.
We may suspend or terminate access for violations of these Terms.
18. Intellectual Property
All Luminum Health names, logos, website content, designs, text, images, program materials, and related intellectual property are owned by or licensed to Luminum Health.
You may not copy, distribute, modify, or use our content or branding without written permission.
19. Disclaimers
Luminum Health services are provided on an “as available” basis. We do not guarantee that the website, patient portal, messaging, video visits, or third-party platforms will always be uninterrupted, error-free, or available.
We do not guarantee specific medical outcomes, eligibility for treatment, prescriptions, lab orders, or program results.
20. Limitation of Liability
To the fullest extent permitted by law, Luminum Health and its owners, employees, contractors, clinicians, vendors, and partners are not liable for indirect, incidental, consequential, special, punitive, or exemplary damages arising from your use of the website or services.
Nothing in these Terms limits rights or protections that cannot legally be waived under applicable healthcare, consumer protection, or privacy laws.
21. Indemnification
You agree to indemnify and hold harmless Luminum Health and its owners, employees, contractors, clinicians, vendors, and partners from claims, damages, losses, liabilities, costs, and expenses arising from your misuse of services, violation of these Terms, or submission of false or incomplete information.
22. Changes to These Terms
We may update these Terms from time to time. The updated version will be posted on our website with a new effective date. Your continued use of Luminum Health after changes are posted means you accept the updated Terms.
23. Governing Law
These Terms are governed by the laws of the State of Washington, unless another state’s laws apply based on your location or applicable healthcare requirements.
